Request FMIG Support

Please complete the form below to request financial assistance for Family Medicine Interest Group activities at a Tennessee medical school. The TNAFP Foundation evaluates every request based on appropriateness, timing, and availability of funds. Staff will respond to the contact provided as soon as a determination is made.

TNAFP will only reimburse funds upon receipt of expenditures up to $500, a summary of the event and photos.

*Please note that the TNAFP Foundation can only grant a limited amount of funds for FMIG support per school, per year. The amount varies depending on donor contributions and other budget factors.

*Appropriate requests include but are not necessarily limited to food for FMIG meetings, speakers or venue rental fees, FMIG member recruitment, or other special projects.
I understand that submitting this request in no way guarantees that that the TNAFP Foundation will approve my request or issue funding. I also understand that, if approved, the TNAFP Foundation requires proof of my related expenditures in the form of receipts, and will ask for photos of any meetings or events to be used in TNAFP/TNAFP Foundation communications.(Required)
